I have a few characters that I like whose chests are often up for multiple interpretations (Kagami Hiiragi, Patchouli Knowledge, etc), but I think my favorite confirmed DFC might be either Ymir from Queen's Blade or Nagi from Kannagi.

My favorite small-chested girl is a tie between Izumi Konata (Lucky Star) and Nakahara Komugi (Nurse Witch Komugi). They are both enjoyable and cute characters, despite their "measurements". ^_^

Same here, I like Koishi Komeiji a lot, but depending on the fanart (ZUN's official art isn't much help), her chest size varies...
As for confirmed DFC's, or ones who are basically confirmed, Menma(AnoHana) and Fuko Ibuki(Clannad) come to mind.
(Seriously, you can't tell me Fuko has a chest under that uniform. Seriously.)
